Start with what has changed

There is no need to prepare a perfect summary. It helps to think about what is different, what feels difficult at home, and what kind of support would make the week more manageable.

  • What has changed since the last doctor visit or hospital stay?
  • Which parts of the day are becoming harder to manage safely or comfortably?
  • Who else should be part of the conversation: a family member, physician, or case manager?

The first call is meant to be a conversation

Our intake team listens first. We can explain the difference between skilled home health and attendant support, answer practical questions, and outline what the next step could look like. If we are not the right fit, you should leave the call with a clearer sense of what to ask next.

Questions worth asking

Good care starts with direct questions. Bring up anything that will affect day-to-day life at home; it is better to talk through it early than make assumptions.

  • What kind of help may fit the situation?
  • What will the first visit or assessment involve?
  • How will the care team communicate with the physician and family?
  • What should we do if needs change?
