Clinical roles
RNs, LVNs, therapists, home health aides, and medical social workers support physician-coordinated care plans in patients’ homes.
RN / LVN
Skilled nursing visits, clinical assessments, care-plan documentation, and coordination in the patient’s home.
Physical Therapy
Provide in-home physical therapy within the individualized plan of care.
Occupational Therapy
Provide in-home occupational therapy focused on daily activities and functional goals.
Speech Therapy
Provide in-home speech therapy for communication and swallowing goals within the plan of care.
Medical Social Work
Help patients and families understand resources and the non-clinical parts of their care plan.
Attendant roles
Attendants provide practical, day-to-day support in clients’ homes. The work calls for reliability, respect for each person’s routine, and close attention to the assigned support plan.
Personal care support
Support personal care routines such as bathing, dressing, grooming, and mobility as outlined in the support plan.
Meals & light housekeeping
Assist with meals, light housekeeping, laundry, and errands as assigned.
Companionship
Provide a dependable, respectful presence while supporting the routines in the assigned plan.
